What to bring to your first visit?

We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite the first appointment, we ask that you please bring your child's immunization records and be prepared to complete paperwork regarding your family medical history, your child's medical history, and insurance information.

Our office uses the most state of the art electronic medical record system, which includes MyChart, our highly functional and user-friendly patient portal. We ask that you complete the MyChart enrollment forms below, and bring them with you to your first visit with us. This will streamline your enrollment into MyChart.

MyChart Patient Portal

Our secure patient portal provides you with on-line access to a wealth of services, including real-time access to test results, school forms, visit summaries, as well as the ability to schedule appointments and message your provider.

What to Expect

On the day of your child's first visit to our office, we request that you arrive 15 minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. We look forward to meeting you and joining with you to help your child maintain optimal health.

8:00 AM - 4:45 PM: In-office and MyChart Telehealth visits
5:00 PM - 6:00 PM: MyChart Telehealth visits only

Sat
8:00 AM - 12:00 PM: MyChart Telehealth visits only

* Please note: It is our policy that we do not approve referral requests for visits to outside urgent care centers during times that our office is open.

Holidays
We are open for urgent visits only on most Holidays. However, our office is closed on the following holidays:

- New Year's Day
- Easter Sunday
- Memorial Day
- July 4th
- Labor Day
- Thanksgiving Day
- Christmas Day

For urgent concerns, you can always reach an on-call provider, even when our office is closed.
